South East mental-health follow-up
The December 2025 findings have not been confirmed as resolved by a later official report in this reference. The bounded check covered CQC's current Defence report directory and targeted CQC/MOD searches on 8 October 2026. No later outcome or closure report was identified. This is a search limit, not evidence that the findings persist today. S45
Known source disagreement
Formal Recommendation 12 gives June 2033 for the pathology training target. Chapter 28, paragraph 31, gives June 2030. This wiki preserves the formal recommendation's date and flags the disagreement. It does not choose a corrected date on the Inquiry's behalf. S02 S30
CQC publication limits
The 2025/26 annual report contains three presentation issues: the medical effective-care denominator is 27 against 11 initial and 15 follow-up inspections; rehabilitation inspections are not reconciled to the headline totals; and the key-findings introduction retains a 2024/25 label. The inspection reference preserves these limits. No corrected total or percentage is inferred. S14
What stays outside
The applicability reference records public scope, not legal entitlement for a particular person. The official ERA section 192 text includes a transitional substitution note and prospective amendments. Their operative effect requires separate checking before legal reliance. MOD's policy extension of PIDA principles does not resolve that question. S23 S15 S43
The public JSP 834 volumes remain linked, but their planned review intervals have passed. DSA03 also identifies a healthcare IT leaflet as temporarily withdrawn. These are reasons to confirm the controlling version, not findings that current safeguards are absent. S16 S12
