Current goal
Provide a public-source reference on the Thirlwall Inquiry, official implementation information and implications for Defence healthcare, with separately identified internal policy questions.
Current state
The initial wiki was published on 7 October 2026 to the private GitHub repository and public Cloudflare static hosting. Initial publication used Git commit c7d694bc8b77dd17ced1a82be40cc1c380e1ef7c and Cloudflare version a0142e18-c6d1-43e3-b7eb-57c39b2b8e31. Public pages and discovery files matched the verified build, and missing routes returned HTTP 404. The managed preview remains available locally.
On 9 October 2026, the owner authorised a move to Cloudflare Pages. The canonical address is now https://thirlwall-wiki.pages.dev/. The owner confirmed that this Pages address works from their closed network, where .workers.dev is blocked. The GitHub repository remains private. Future uploads target Pages, with the previous Workers deployment retained as a fallback.
The reference covers all 17 recommendations, 49 registered sources and 28 traceable evidence claims. The public implementation tracker records responses, published directions and delivery limits as at 7 October 2026. The internal policy reference retains one applicability entry and 17 recommendation-specific entries.
The first complete verification run passed on 7 October 2026. It covered the build, content and source links, schemas, graph, desktop and mobile layouts, managed service and repository provenance. A final run records the completed handover state in output/verification/receipt.json and output/verification/conformance-report.json.
The legal applicability reference distinguishes legislation, Defence regulation, professional standards and policy protection. The 9 October expansion adds responsible roles, triggers and required responses for existing Defence duties. It makes command direction, change management and complaints explicit alongside safety, reporting, candour, staffing and governance. The enforcement reference separates CQC inspection from DMSR enforcement and records the limit on public evidence about formal closure authority. Internal questions and E23–E25 trace the comparison.
The CQC inspection reference now records the 2025/26 inspection groups and findings, with service and period limits, precise locators and provisional Defence relevance. The primary Cyprus report now supplies its five recommendations, inspection date, ratings and unverified later resolution. The South East primary report now adds exact December 2025 visit dates, treatment waits, reported causes, safety and leadership concerns, and positive findings. Its later resolution was not established in the bounded search on 8 October 2026. Source and follow-up limits are recorded in VAL-008 and VAL-009.
Home now gives an Executive Summary before the reading routes. The navigation separates implications, Defence reference material, sources and maintainer information. Long legal and inspection tables use short sections, and the detailed recommendations and policy questions have topic indexes. The reader review records the structural and language findings.
The whole-wiki review on 9 October 2026 covered all 23 Wiki pages and eight underlying knowledge pages. It corrected three inspection links and clarified the initial review date. The review found no need for a broad structural or language rewrite. It did not recheck external publications or current law.
The later presentation review on 9 October followed the owner's request for clearer visual grouping. It added short navigation cards, section bands and open reference panels to the long topic pages. The source register retains its native rows with revised column widths. The review record distinguishes this work from the earlier language review.
The death-duty reference now covers coroner notification, certification, medical-examiner scrutiny and child-death review in England. It identifies responsible roles, triggers, recipients and exceptions. Sources S46–S50 and claims E26–E28 support the addition. CHECK-08 and CHECK-11 retain the separate questions about internal arrangements. This is not a UK-wide or overseas legal survey.
The targeted inspection expansion on 9 October links the existing Cyprus and South East findings to precise staffing, reporting and governance duties. It uses the registered historical reports and does not reopen the parked closure searches. The mapping is provisional and does not establish a breach of the mapped clauses.
The assessment remains provisional. It does not establish a Defence policy gap or nationwide implementation. The user selected a reference wiki: no internal pilot, invented scenarios or worked examples. The validation queue records these limits and the conflicting dates for Recommendation 12.
The configurable appearance contribution has completed its technical handoff to the Standard. Integration commit dc14307508a24b170c7401f9469a1ee83115ec1f records successful verification, including 183 tests and 13 appearance cases. The proposal guide distinguishes that completed work from formal intake, acceptance, release and adoption. The later article-link colour improvement remains a separate local change. The wiki's pinned Standard revision is unchanged.
The user-confirmed content-development pattern applies after every development cycle. The local Standard implementation now carries that rule through its instructions, starter, plugin and checks. The content-development proposal records its consumer origin and implementation evidence. Technical implementation must not be proposed again merely because formal intake or release is pending.
Proposed content priorities
The user requested content-development suggestions after each development cycle. These are bounded refinements to the continuing reference, not claims that the initial 17-recommendation review is unfinished. They are recommendations for selection; no new research or internal review is authorised by this list alone.
The requested duty, enforcement, targeted-inspection, England death-duty and presentation expansions are complete. The five-theme summary remains the short overview.
- Proposed update: Add Scotland's public death-reporting duties to Rules and responsibilities, linked to CHECK-11. Why it matters: The audience is UK Defence healthcare, but the detailed external death-duty reference currently covers England. Done when: Current Scottish primary sources establish the responsible roles, reporting triggers and recipients, with a short comparison and separate internal questions. Keep child-death review, Northern Ireland and overseas law outside this single pass.
Technical verification limits
The Standard Production report concerns this local implementation. It is not a certification of the research conclusions or a public conformance release. The npm audit passed its required moderate-severity threshold, with two low-severity findings in the pinned Mermaid/KaTeX dependency chain. The Python dependency audit reported no known vulnerabilities.
