2026-10-09

  • England death duties: Added current public rules for coroner notification, certification, medical-examiner scrutiny and child-death review. Registered S46–S50 and E26–E28. Kept doctor, examiner and review-partner responsibilities distinct, including the difference between practitioner notification guidance and a statutory information request. Extended CHECK-08 and CHECK-11 without claiming internal validation or delivery of the Inquiry's proposed changes. Earlier legal topics were not rechecked.
  • Presentation review: Reviewed all 23 Wiki page structures and inspected eight desktop views across seven main reading routes. Added short navigation cards, major section bands and open panels for repeated entries. Retained native headings, citations, R/CHECK anchors and full visible text. Revised the source table's column widths. The review record records the tools, design references and review limits.
  • Content priority: Completed the previously proposed England death-duty addition. Proposed a separate, bounded Scottish death-reporting comparison because the wiki's audience is UK-wide. It is not authority for further research or an internal review.

  • Existing duties and enforcement: Expanded Rules and responsibilities to identify the actor, trigger, required response, precise clause and separate internal question. Rechecked the DMSR landing page, DSA02 and regulatory-notification page. Added command direction, change management and complaints, and made existing safeguarding, reporting, candour, staffing and governance duties explicit. Distinguished CQC inspection, DMSR enforcement and unverified formal closure authority. Recorded that internal boundary in VAL-010. The earlier statutory analysis was not refreshed.

  • Targeted inspection comparison: Related existing Cyprus and South East findings to staffing, reporting, risk ownership and effective corrective action. Used the registered report copies with matching digests; live CQC PDF retrieval was unavailable. Preserved ratings, positive findings and service/date limits. This pass did not repeat the parked later-resolution searches or find a breach of the mapped clauses. Added E23–E25 and linked the Human and LLM reference material.
  • Pages access confirmed: The owner confirmed that the Pages site works from their closed network.
  • Next content priority: Proposed a bounded public-law reference on death notification and child-death responsibilities in England, linked to CHECK-08 and CHECK-11. This remains a recommendation, not authority to perform a separate internal review.

  • Pages hosting: Moved the publication configuration and canonical address to Cloudflare Pages at the owner's request. Future uploads use the existing Pages project and its production branch. The repository remains private, and the previous Workers deployment is retained as a fallback. Added an offline deployment preflight. This hosting change does not change topic evidence or source check dates; access from the owner's closed network remains a separate check.

  • Whole-wiki reader review: Reviewed all 23 Wiki pages and eight underlying knowledge pages for structure, language and consistency. Corrected three inspection links that led to a neonatal-only reporting check. Clarified the initial review date in the research method. No broad rewrite was needed. The review record gives the coverage and limits; no external source was newly checked.
  • Priority review: Retired the proposed five-theme summary rewrite because the review found no material reader gap. The rewrite was judged unnecessary, not completed. Further expansion is parked until new evidence or an explicit request warrants it.

2026-10-08

  • Home focus: Removed "About this reference" and its generic architecture diagram at the user's request. Home now ends with the subject scope. The architecture explanation remains on its separate page.
  • Sidebar simplification: Removed the repeated draft-status icons from visible navigation at the user's request. Source status metadata remains intact. Verified the page links and section controls in both appearances and at desktop and mobile widths.
  • Content direction corrected: Withdrew the Waddington inspection addition as the next priority. The next proposed update is a direct comparison of Inquiry implications, existing legal and regulatory requirements, and specific internal policy questions. More inspection detail is not the current priority.

  • South East mental-health primary report: Registered S45, published 20 March 2026. Recorded the three December 2025 visits, all ratings, treatment waits and their reported causes, safety and leadership concerns, and positive findings. Preserved the distinction between longest and average waits, existing monitoring and unclear risk ownership, and inspection findings and present performance. Added E22, the source digest and the assurance-concept link. No source PDF is retained in the repository.

  • Follow-up boundary: Checked CQC's Defence report directory and targeted CQC/MOD searches for a later South East outcome or closure report. None was identified. Parked further searches in VAL-009 until new evidence or an explicit request. The inconsistent referral percentage at p.7 is not used. Verified CQC's general reuse terms and retained attribution, third-party exclusions and logo restrictions.
  • Reader dates and handoff: Updated the Home summary and distinguished the latest content date from the initial source review. Existing source check dates remain unchanged. Completed the South East content priority; proposed a bounded primary-source addition for the annual account's Waddington dental finding.

2026-10-07

  • Reader structure and language: Added an Executive Summary to Home, three main reading routes and topic indexes for recommendations and internal questions. Grouped navigation by reader task. Separated inspection findings from rules and responsibilities, converted long prose tables to labelled sections, and made internal questions individual bullets. Preserved source identities, recommendation/check anchors and legal qualifications. The reader review records the findings and limits.
  • Cyprus primary inspection: Registered S44, the report published on 24 February 2026 after inspection on 9–11 December 2025. Added its five recommendations with precise page locators. Retained Good overall alongside Requires improvement for safety, positive changes and unverified later resolution. Recorded CQC's working-hours finding without determining an employment-law breach. Added E21 and the public PDF digest; no report copy is retained in the repository.

  • CQC inspection reference: Expanded the public comparison with inspection groups, service findings, precise section locators, limits and provisional Defence relevance. Registered the annual report's relevant subpages and linked the assurance concept, evidence claims and separate internal questions. Retained positive findings and unresolved concerns. Recorded source inconsistencies without inferring corrected counts or implementation of the later Inquiry recommendations.

  • Green accent restored: At the user's request, replaced the teal text and link colours with the darker sage family derived from #e1efdd. Light-mode links and accents use #505c4d; dark-mode links use the pale-green family. Kept link underlines and visible focus. Aligned the design contract, favicon, graph controls and existing visual checks.

  • Legal applicability: After initial publication, expanded the public comparison with seven entries separating legislation, Defence regulation, supporting guidance, professional standards and policy protection. Verified official provisions and current-linked regulator documents. Registered the ERA protected-disclosure framework as S43, added precise scope references to existing records, and refined E03–E07. Linked the separate provider and reporting questions without determining internal coverage or individual legal rights. Preserved section 192 commencement and transitional uncertainty. The CQC inspection account is unchanged.

  • Teal accent: The user selected #03658c for links and the darker accent. Applied it to light-mode links, controls, icons, selected-navigation text, question rails and graph interactions. Visited links retain the same teal instead of purple. Dark-mode links use readable tints of that teal. Pale-green surfaces, light appearance and wide-layout defaults are retained.

  • Content development: The user requested content next-step suggestions after each development cycle. The user confirmed the pattern: up to three priorities, each stating the proposed update, why it matters and what counts as done. Recorded that standing preference and proposed two bounded reference refinements: legal applicability and CQC inspection findings. Initial recommendation coverage is complete, but the continuing reference can be developed further. Replaced contradictory local guidance and implemented the reusable rule in the local Standard, including shared instructions, starter templates, plugin and captured-response checks. Formal intake and release remain separate. See the proposal guide.

  • Status correction: Verified Standard integration commit dc14307508a24b170c7401f9469a1ee83115ec1f and removed the completed technical handoff from active priorities. Formal acceptance, release and adoption remain separate pending transitions. The later article-link colours were not included in that integration.
  • Evidence presentation: Rejoined E13–E17 to the evidence table. Their claims and citations are unchanged.

  • Reference scope: User selected information-only topic content and declined an internal pilot. Replaced scenarios, exercise instructions and review-programme priorities with policy areas, factual questions and evidence distinctions. Retained all CHECK-00–17 anchors and the original governing question.

  • Public implementation: Added a dated tracker for all 17 recommendations, with official recipients, milestones, responses, published instruments and delivery limits. Registered the initial government response, NHS England PRN02660 and relevant earlier instruments. Recorded the published SUDIC clarification as partial delivery; did not infer Trust compliance or a new Defence duty.
  • Current naming: Recorded the Ombudsman name change effective 1 October 2026 and its explicit unchanged service scope. Retained the report's historical recipient name and did not treat the rename as R16 implementation.

  • Standard proposal: Prepared configurable-appearance-family from the verified and committed consumer fixes. A local Standard candidate now provides reusable colour derivation and configurable appearance and width defaults. The consumer example retains #e1efdd, dark sage #505c4d, light appearance and wide desktop layout. Explicit saved reader choices take precedence. Candidate commit 3ec5e3c5 passed distribution verification, 182 tests, the complete browser contract and nine appearance cases. A separate configured full-browser run and maintainer provenance check passed. No managed service was created for the isolated candidate. Exact payload approval, Standard review, acceptance, release and consumer adoption remain pending. No issue was submitted, and the Standard pin is unchanged. See the proposal record guide.

  • Darker accent: Replaced the strong dark green with #505c4d, a dark sage derived from #e1efdd. Matched the link states, focus indicators, icons and graph controls. Retained readable contrast.

  • Reader defaults: Set wide layout and light appearance as the wiki defaults. Keep explicit reader choices of standard width or dark appearance. Mobile layout remains responsive.

  • Style correction: Reviewed the rendered light, dark and graph pages after the user identified that the chosen pale green was barely visible. Applied #e1efdd to visible surfaces, retained readable dark text, aligned supporting colours, enlarged table text and narrowed citation decoration to source identities. Added checks for the actual requested colour.
  • Handover: Completed the initial assessment, all 17 recommendation reviews and the separate internal-check specification. The first complete local verification passed. Final generated receipts record the handover build and checks; they do not validate internal Defence policy or certify the research conclusions.
  • Verification review: Added checks for source-row provenance and the exact inherited Standard profiles. Reviewed source fidelity independently and inspected the rendered wiki. Kept research limits in the validation queue.
  • Scope confirmed: Defence healthcare, with wider Defence lessons where relevant.
  • Evidence boundary confirmed: Public sources and provisional implications may be included. Non-public Defence policy and internally validated findings remain outside the wiki.
  • Reader task confirmed: Identify specific matters to check against internal policy in a separate review.
  • Source work: Read the formal recommendations and supporting report chapters. Examine current-linked official Defence, legal and professional sources. Record version and scope limitations.
  • Source identity: Preserved SHA-256 fingerprints for eight selected public PDFs in config/public-source-fingerprints.json. No source PDFs were retained in the repository.
  • Analysis: Cover all 17 recommendations. Distinguish NHS recipients, legal applicability, existing public comparators and proposed Defence review actions.
  • Independent review: Clarified that R8 notification deadlines concern circulating guidance, not reporting individual deaths. Corrected incident-escalation citations to the controlling Defence regulatory articles.
  • Uncertainty: Preserve the R12/chapter 28 date discrepancy in the validation queue. Do not infer policy gaps from unavailable internal material.
  • Design: User selected pale green #e1efdd. Use accessible related dark colours for text and controls.
  • Standard: Pin the DenchCo Standard to d339a0e30de41c4a3260f22c00c96898299a1834. Use the Standard Production profile and a local-only managed wiki.
