Source register
This register records the public sources used in this wiki. The initial subject-source review was on 7 October 2026. Each row records its own check date, including later additions. Technical provenance dates are separate.
An official page can remain online after its content changes. The date below records the version inspected, not guaranteed future currency. “Current public copy” means the responsible authority still linked that copy when checked. It does not certify the latest internal Defence version.
The Inquiry report establishes findings and recommendations. It does not establish that recommendations became law or that Defence adopted them. Publication dates, policy version dates and legal commencement dates have different meanings.
Use a numbered source link, such as S44, to jump to its exact record. For the conclusions rather than the source catalogue, read the Home summary or Inspection findings.
Inquiry evidence
All chapter entries belong to the report presented to Parliament on 15 September 2026. The report hub supplies that date. Chapter pages have no separate amendment history. Read the numbered paragraphs or recommendations identified by the citing page.

England and Wales legislation; this comparison addresses England. The latest page reports no known outstanding effects, subject to the publisher's editorial-update limits. SI 2024/668 regulations 1, 4 and 5 establish the September 2024 change and transition. Historical cases require the transitional provisions. |
| S47 | Medical Certificate of Cause of Death Regulations 2024; official XML; 2024 amendment | UK legislation / The National Archives | SI 2024/492; commenced 9 September 2024; revised text valid from 22 November 2024; XML modified 13 January 2025 | 2026-10-09 | Applies to deaths required to be registered under Part 2 of the Births and Deaths Registration Act 1953. England and Wales legislation; regulations 3, 6–11 and 14–20 supply certification, scrutiny and referral duties; regulation 26 preserves transition rules. The latest page reports no known outstanding effects, subject to editorial-update limits. SI 2024/1064 inserted the information-sharing provision in regulation 25A and corrected forms. |

Regulations 6–7 address independence, serious concerns and information for child-death reviews. The official endpoint supplies as-made text; the changes search returned no recorded effects at this check. That result remains subject to editorial-update limits and is not a consolidated-text claim. |
| S49 | Children Act 2004, section 16M; section 16N; section 16Q; section 16M official XML; section 16N official XML; section 16Q official XML | UK legislation / The National Archives | 2004 c.31; current section XML modified 28 September 2026, consolidation valid 30 September 2026; these child-death provisions commenced 29 June 2018; ICB substitution effective 1 July 2022 | 2026-10-09 | England child-death review arrangements and requested-information duties. Section 16Q identifies the local authority and overlapping integrated care boards as partners. Official XML lists outstanding effects elsewhere in the Act, but none names sections 16M, 16N or 16Q. Do not infer the whole Act is free of outstanding changes. Normal residence, provider role and the terms of a section 16N request remain material. |
| S50 | Working together to safeguard children; 2026 statutory guidance | Department for Education | March 2026; publication page updated 18 March 2026 | 2026-10-09 | Current England statutory guidance, replacing the 2023 edition. Chapter 6, paragraphs 433–440 and 447, pp.151–152 and 156, distinguish review arrangements, information requests and practitioner notification. Paragraph 447 says practitioners should notify through the local Child Death Overview Panel administrator or equivalent. Preserve this guidance wording and the statutory partners' section 16Q duty to have regard to guidance. Not proof of local implementation or delivery of R8/R11. |

Rights and retention
Public access does not establish unrestricted reuse. These rules apply to each source in the stated category. The wiki retains source identity and limited original paraphrase. It does not republish full reports, policies or professional codes. Third-party rights remain with their owners.
| Source category | Rights and retention treatment |
|---|---|
| Inquiry: S01–S09, S30–S35 | Official pages identify Crown copyright. Reuse terms for incorporated evidence were not separately verified. Retain links and concise analysis; do not reproduce underlying witness material or complete chapters. |
| MOD: S10–S13, S15–S19, S24–S26 | GOV.UK pages state Open Government Licence v3.0 except where otherwise stated. Check attachment and third-party exceptions before reuse. This wiki retains citations and paraphrases; it does not mirror policies. |
| CQC: S14, S44, S45 | CQC's terms, checked 8 October 2026, permit reuse of its material under the Open Government Licence, with third-party material excluded and separate logo restrictions. Contains Care Quality Commission information licensed under the Open Government Licence v3.0. This wiki retains links and original summaries; it does not republish reports or logos. |
| Professional regulators: S20, S21 | Retain official links, clause references and limited paraphrase. Do not assume a government licence applies or reproduce the full standards. |
| Legislation: S22, S23, S43, S46, S47, S48, S49 | Retain official text links, provision identities and version metadata. XML was used for verification, not republished here. Check applicable terms before distributing text or data extracts. |
| Statutory guidance: S50 | GOV.UK states Open Government Licence v3.0 except where otherwise stated. Retain source links, precise paragraph references and concise paraphrases; do not republish the guidance. |
| Response sources: S36–S42 | Retain links, dates and concise factual summaries. GOV.UK and NHS England reuse terms apply to their material; the Ombudsman page retains its own copyright. No complete source reproduction or source-file publication. |
| Technical references: S90, S91 | Retain immutable identities, hashes and provenance. Any imported technical files remain subject to their upstream licences and the repository's licensing record. |
If a source is withdrawn, corrected or removed, review dependent claims and preserve the reason for their changed evidence state. Do not silently substitute a cached copy as current authority. Any later retained source copy needs a recorded purpose, access rule and removal decision.
Verification limits
The register does not import non-public Defence policy. Public references to internal documents identify later verification work only. Older copies, search excerpts and remembered policy content cannot fill that evidence boundary.
The legal review is bounded. Devolved legislation, overseas jurisdiction, NHS-hosted placements, contractors and employment status require setting-specific checks. No claim here certifies legal compliance.
The Rules and responsibilities separates published requirements from inference. The internal policy checks identify questions for an authorised environment.
