# Validation queue — open questions and limitations

The wiki records unresolved questions without treating them as deficiencies in Defence policy. Internal findings, pilot work and worked examples are outside this reference.

| ID | Question | State | Bounded action | blocked_by | reopen_when | last_checked |
|---|---|---|---|---|---|---|
| VAL-001 | Do current internal policies and local procedures cover the identified policy areas? | Outside reference scope | Retain the public questions and evidence boundary; no internal pilot is planned. | User selected information-only reference scope | A separate internal task is explicitly requested | 2026-10-07 |
| VAL-002 | Which requirements apply to each provider, location and staff group? | Public boundaries recorded; service-specific application parked | Use the [legal applicability reference](https://thirlwall-wiki.pages.dev/defence-comparison/index.md#legal-applicability-reference) to frame separate advice. Preserve ERA section 192 commencement and transitional questions. | Service-specific facts and legal determinations are outside this public reference | A particular service is selected for internal review, or relevant official legal change is published | 2026-10-07 |
| VAL-003 | Do public versions match the current internal issues? | Parked | Ask the relevant document owner to confirm operative versions outside the wiki. | Internal versions outside scope | Approved public version or authorised separate confirmation is available | 2026-10-07 |
| VAL-004 | Which recommendations have received a formal government response, legal change or implementation evidence? | Dated public status recorded; delivery limits remain | [Tracker](https://thirlwall-wiki.pages.dev/implementation/index.md) covers all 17, distinguishing commitment, direction and delivery. | A bounded public check cannot establish nationwide compliance; a fuller government response was not identified | New official evidence or an explicit request to refresh the status | 2026-10-07 |
| VAL-005 | Why do R12 and chapter 28 give different pathology workforce dates? | Open source discrepancy | Seek an official correction or clarification; retain formal R12 date meanwhile. | The report's stated dates differ | Inquiry correction or clarified official response appears | 2026-10-07 |
| VAL-006 | Have the public JSP 834 volumes completed their stated review cycle? | Public currentness unresolved | Check for an updated official publication before relying on its issue status. | Publicly linked volumes do not establish internal review status | An updated public version or approved clarification appears | 2026-10-07 |
| VAL-007 | What has replaced the healthcare IT leaflet marked temporarily withdrawn in DSA03? | Parked | Ask the document owner to identify the operative instrument in the separate internal review. | Internal replacement status is not public evidence here | An official public replacement is published | 2026-10-07 |
| VAL-008 | How should CQC's medical denominator, rehabilitation totals and landing-page year label be reconciled? | Source inconsistencies recorded; reconciliation parked | Preserve the source figures and [specific limits](#cqc-publication-limits); do not derive a disputed rate. | The annual account does not reconcile these statements | CQC publishes corrected text or an official clarification | 2026-10-07 |
| VAL-009 | Has a later official report confirmed resolution of the South East mental-health findings? | Bounded search completed; follow-up parked | Retain the inspection date and [search limit](#south-east-follow-up); do not infer present performance. | No later outcome or closure report identified in the checked official sources | A new official outcome report appears, or the user explicitly requests a fresh search | 2026-10-08 |
| VAL-010 | Who formally accepts closure of each DMSR corrective requirement or enforcement notice? | Public roles recorded; internal procedure outside reference scope | Retain the [public enforcement boundary](https://thirlwall-wiki.pages.dev/defence-comparison/index.md#inspection-enforcement-and-closure) and [CHECK-17](https://thirlwall-wiki.pages.dev/internal-policy-checks/index.md#check-17). | DSA02 identifies the detailed corrective-action and enforcement guide as Defence Intranet only | An authoritative public procedure is published, or a separate internal task is explicitly requested | 2026-10-09 |

## South East mental-health follow-up

The December 2025 findings have not been confirmed as resolved by a later official report in this reference. The bounded check covered CQC's current Defence report directory and targeted CQC/MOD searches on 8 October 2026. No later outcome or closure report was identified. This is a search limit, not evidence that the findings persist today. [S45](https://thirlwall-wiki.pages.dev/sources/index.md#s45)

## Known source disagreement

Formal Recommendation 12 gives **June 2033** for the pathology training target. Chapter 28, paragraph 31, gives **June 2030**. This wiki preserves the formal recommendation's date and flags the disagreement. It does not choose a corrected date on the Inquiry's behalf. [S02](https://thirlwall-wiki.pages.dev/sources/index.md#s02) [S30](https://thirlwall-wiki.pages.dev/sources/index.md#s30)

## CQC publication limits

The 2025/26 annual report contains three presentation issues: the medical effective-care denominator is 27 against 11 initial and 15 follow-up inspections; rehabilitation inspections are not reconciled to the headline totals; and the key-findings introduction retains a 2024/25 label. The [inspection reference](https://thirlwall-wiki.pages.dev/inspection-findings/index.md) preserves these limits. No corrected total or percentage is inferred. [S14](https://thirlwall-wiki.pages.dev/sources/index.md#s14)

## What stays outside

The applicability reference records public scope, not legal entitlement for a particular person. The official ERA section 192 text includes a transitional substitution note and prospective amendments. Their operative effect requires separate checking before legal reliance. MOD's policy extension of PIDA principles does not resolve that question. [S23](https://thirlwall-wiki.pages.dev/sources/index.md#s23) [S15](https://thirlwall-wiki.pages.dev/sources/index.md#s15) [S43](https://thirlwall-wiki.pages.dev/sources/index.md#s43)

The public JSP 834 volumes remain linked, but their planned review intervals have passed. DSA03 also identifies a healthcare IT leaflet as temporarily withdrawn. These are reasons to confirm the controlling version, not findings that current safeguards are absent. [S16](https://thirlwall-wiki.pages.dev/sources/index.md#s16) [S12](https://thirlwall-wiki.pages.dev/sources/index.md#s12)

The [internal policy reference](https://thirlwall-wiki.pages.dev/internal-policy-checks/index.md) records questions and relevant policy areas. Internal answers, extracts and evidence remain outside this wiki. A check cannot be closed here by assuming that no published material means no existing safeguard.

Parked items are not automatic follow-up tasks. A future update requires the stated trigger or an explicit user request. See the [research method](https://thirlwall-wiki.pages.dev/approach/index.md) and [project status](https://thirlwall-wiki.pages.dev/project/status/index.md).
